Orthotics provide stimulus underfoot to improve foot function to reduce load and allow soft tissue to heal as un-hindered as possible
Orthotics Reduce Soft Tissue Load
To aid healing injured soft tissue structures of the feet and lower limbs we need to be able to take enough load away from it to allow the body’s self-healing abilities to work as un-hindered as possible.
Ideally a reduction in load is done while maintaining as normal gait as possible as opposed to wearing a moonboot that restricts ankle movement and alters normal gait. The later which can cause atrophy of surrounding soft tissue leading to other conditions and/or a longer return to your chosen activity if not managed properly.
Below are the different structures of the body that can be susceptible to an overuse injury as well as common conditions we see as Podiatrists.
Please click on your affected body structure your Podiatrist has identified to find out how Orthotics will help as part of an overall treatment plan.
Common conditions:
- Plantar fasciitis
- Plantar heel pain
- Arch pain
The plantar fascia is a ligament of the medial longitudinal arch of the foot that plays an important functional role of stabilising the bones during the stance and propulsion phase of walking and running gait. You can think of this structure as a “cable” that is placed under stretch forces when we weight-bear.
If there is in-efficient foot function (e.g. excessive flattening and therefore lengthening of the foot) this can place un-due, repetitive stretch forces upon the plantar fascia during walking or running potentially contributing to a very common overuse injury called plantar fasciitis.
The role of Orthotics in helping heal plantar fasciitis is to improve foot function and to safely manipulate the bones of the feet and ankles to achieve shortening of the plantar fascia. By doing this the plantar fascia is less irritated on each stride and in combination with other modalities this will aid in healing the condition over time.
Common conditions:
- Posterior tibial tendinopathy
- Posterior tibial tendon dysfunction
- Adult acquired flat foot deformity
- Peroneal tendinopathy
- Flexor hallucis longus tendinopathy
- Achilles tendinopathy
- Shin splints or Medial tibial stress syndrome
- Sever’s disease
When trying to understand the role that muscles and tendons play during movement you can also think of them as “cables” that shorten and elongate to move the body. Tendons are the extension of muscles that insert into bones to create movement once muscles contract.
To role of Orthotics to aid healing affected muscles or tendons is to improve the efficiency of how these “cables” shorten and elongate. If they are more efficient, they do work as hard. This leads to a reduction in soft tissue load to better allow the body’s self-healing processes to take effect.
Common conditions:
- Mid-foot arthritis
- 1st Metatarsal-phalangeal joint (Big toe joint) arthritis
- Rearfoot and ankle joint arthritis
- Plantar plate tears
- Metatarsalgia
Joints form the connection between two or more bones. There are 33 joints in each foot that allow bending to help us move over the undulating surfaces we have evolved to place our feet on – grass, dirt and sand for example.
Joints of the feet have various structures that provide stability between the bones while moving. These include ligaments and a fibrous joint capsule.
If there is trauma, over-use, anatomical restriction and/or aging wear processes (e.g. arthritis) to any of these joints and associated structures this can be a source of chronic pain.
The main role of Orthotics in this case is to reduce bending forces at joints. Less bending the better the bodies self-healing processes can occur. If it is a joint that also contacts the ground (e.g. ball of the foot joints) Orthotics are also deflecting and reducing pressure under the joint to aid in healing.
Matched with the appropriate stiff soled, stable footwear this completes the joint de-loading picture and in combination with other modalities helps rehabilitation.
Common conditions:
- Morton’s neuroma
- Intermetatarsal bursitis
We have nerves that innervate to the feet that provide muscle activation, body responses and sensation.
Bursae provide protection between either bone on tendon or between bone and bone.
Both are susceptible to compression in different areas of the foot and ankle because they run intimately with other structures. The effects of tight or ill-fitting footwear are often the main protagonist.
The role of Orthotics in helping irritated, inflamed nerves and bursae is to deflect pressure away from them and to de-compress them by safely spreading the adjacent structures.
This combined with the correct wide fitting shoes as well as good footwear habits completes the de-loading solution as part of the rehabilitation plan.
Common conditions:
- Sesamoiditis
- Sesamoid bone stress injury
- Metatarsal bone stress injury
- Navicular bone stress injury
- Frieberg’s disease
There are 26 bones in each foot, one-quarter of all of the bones in the body are in the feet. Bones can be susceptible to damage due to high ground reaction forces from repetitive activities such as running and even exercise walking. This especially if bones have not had enough time to develop a high enough bone density for the activity.
The most common contributing factor that can lead to a bone stress injury is doing “too much, too soon”. Having a graduated exercise program can help with reducing this risk factor. Other contributing factors can include lack of nutrients, sex (F>M), flat feet, high arches, previous stress fractures, inappropriate footwear and weakened bones (from conditions like osteoporosis).
The role of Orthotics in rehabilitation of bone stress injuries is to reduce ground reaction forces and bending stress on them to help them heal along normal physiological time frames.
This may be after time is spent in a moon boot (if required), to help return to normal walking gait as soon as appropriate. They can then also play a part in reducing the chance of re-occurrence once normal chosen activity is resumed with guidance from your practitioner.
Common conditions:
- Anterior knee pain
- ITB friction syndrome
- Medial knee osteoarthritis
- Pathologies of the hip and pelvis
- Lower back pain
The movement of the feet affect further up the kinetic chain. The kinetic chain describes each of the segments of the lower limb and pelvis (toes, feet, ankles, lower legs, knees, upper legs, hips and pelvis) being connected by ligaments at their joints. Movement of one causes movement of the others.
During movement if there is excessive movement of the feet, there is potential for excessive rotation forces being applied to the lower limbs, pelvis including the lower back (which is connected to the pelvis). This can lead to contribution to an overuse injury or continual irritation of existing painful structures all along the chain.
Orthotics help improve efficiency of foot movement to reduce rotation forces at the lower limbs.
This treatment is typically done in combination with the appropriate strength and conditioning program. In combination they can both aid in your rehabilitation and also help in reducing the risk of the condition returning.

He enjoys bushwalking and hopes to spend more time with his wife and two young girls exploring The World’s natural wonders.
